Safety acknowledgement

I understand that exercise involves a risk of injury. I agree to follow staff instructions, use equipment responsibly and avoid reckless behaviour.

I declare that I have completed the appropriate medical examinations and have been advised that I can participate in physical exercise. I will inform BaseTYL staff if this changes or if I experience symptoms that make exercise unsafe.

I understand that I am responsible for following the gym’s safety rules. Nothing in this acknowledgement removes rights or responsibilities that cannot legally be excluded.
